WHAT
IS THE ROCHE DISASTER POLICY?
A long-standing Roche service (applies to Roche Laboratories Inc., Syntex
Laboratories Inc. and Roche Products Inc. products) that extends help
to wholesalers, pharmacies and hospitals that are the victims of fire,
flood, storms, earthquakes, civil disturbances, or other catastrophes.
WHAT
SHOULD I DO IF DISASTER STRIKES MY PHARMACY?
Contact your Roche representative, who will notify the Order Fulfillment
Organization that a disaster has occurred. If
you prefer, you may call the Order Fulfillment Organization directly at
800-526-0625. In either case, be
prepared to provide the following information:
Your current business address, whether you are at your original site or an alternate site.
The name and the phone number of the contact person at your current operating location.
A copy of your DEA certificate.
WHAT ASSISTANCE IS AVAILABLE
FROM ROCHE?
You may choose to receive a ROCHE DISASTER PAK or credit from your
wholesaler for the return of the unsaleable or damaged goods. If you elect
to receive a DISASTER PAK, you will be asked for a list of the
Roche products for which you fill weekly prescriptions.
WHAT
DOES THE ROCHE DISASTER PAK CONTAIN?
The Roche products you listed will be supplied in 1 x 100 quantities or
less, depending on the product's unit size.
IF YOUR CURRENT OPERATING ADDRESS IS NOT THE BUSINESS LOCATION REGISTERED WITH THE DEA, ROCHE WILL NOT INCLUDE CONTROLLED SUBSTANCES IN THE DISASTER PAK.
HOW SOON WILL I RECEIVE
MY DISASTER PAK?
You will receive your DISASTER PAK within a week after you have
filed your claim with Roche. After your DISASTER
PAK has been shipped, the Order Fulfillment Organization will phone
you and your Roche representative to confirm that you have received your
DISASTER PAK and that action has been taken on your disaster claim.
| WHAT
ABOUT MERCHANDISE DAMAGED BY FIRE OR FLOOD? Damaged merchandise should be returned to Capital Returns immediately. If your merchandise has been condemned by a public health official, proof of seizure and destruction of the damaged merchandise should be provided. Losses of controlled substances must be reported to local DEA officials, and you must submit a copy of the report along with the statement by the public health official making the seizure. |
