Description
In the United States, people receive anticoagulants for various indications such as prevention and treatment of venous thrombosis, prevention of embolism with prosthetic heart valves, atrial fibrillation, or ischemic cerebrovascular disease. Regardless of the anticoagulant prescribed, it is important for pharmacists to be aware of products available and the appropriate monitoring parameters of these medications to ensure desired therapeutic outcomes and avoid adverse effects.

Learning Objectives
Upon successful completion of this activity, the pharmacist should be better able to:

  1. Understand pharmacokinetic characteristics that may affect the management of anticoagulant therapy.
  2. List important issues to discuss when counseling patients about anticoagulant therapy.
  3. Identify potential drug interactions with warfarin therapy.
  4. Recommend appropriate monitoring parameters for anticoagulants.
  5. Recognize therapeutic ranges and proper management of over-anticoagulation.

Target Audience
The target market for this program is pharmacists.

  • Successfully complete the post-test (70% or higher).
  • You will have two (2) opportunities to successfully complete the activity.

Your statement of credit will be issued immediately upon successful completion of the post-test and submission of the evaluation.
