Description

Chronic kidney disease is reaching epidemic proportions in the United States, and community practitioners are key to the early identification of these patients and those at risk for the development of CKD.

Learning Objectives
Upon successful completion of this activity, the pharmacist and nurse should be able to:

  1. Describe the epidemiology and costs associated with Chronic Kidney Disease (CKD).
  2. Identify risk factors and primary indicators for CKD.
  3. Use prescription medication profiles to identify patients at risk for the development of CKD.
  4. Articulate the need for routine screening and of screening methods, and the importance of early referral for diagnosis.
  5. Briefly overview strategies for prevention of/slowing progression of kidney disease.
  6. Outline the potential complications and comorbidities of CKD.
  7. Outline the clinical practice guidelines relevant to kidney disease.
  8. Understand the role of community health professionals in medication therapy management programs in CKD.

Target Audience
Pharmacists, pharmacy technicians, and nurses.

CE Credit
To receive credit for this program, you must:

  • Review the full content of the program and reflect on its teaching.

  • Successfully complete the post-test with a minimum of 11 out of 15 correct answers.

  • You will have two (2) opportunities to successfully complete the program.
Your statement of credit will be issued immediately upon successful completion of the post-test and submission of the evaluation.
