Description

As community pharmacy practices shift from a product-centered to a patient-centered focus, pharmacists' ability to provide medication therapy management (MTM) services will be vital as the profession evolves. Sound planning and quality assurance will ensure that the service that your pharmacy offers will stand the test of time. Traditionally, pharmacists have been regarded as the medication experts. Therefore, community pharmacists are in ideal settings to refine and utilize this unique skill that patients and providers have grown to depend upon. MTM services provide a novel means to do just that.

Learning Objectives
Upon successful completion of this activity, the pharmacist should be better able to:

  1. Define medication therapy management (MTM) services.
  2. List the steps necessary to implement an MTM program in the community pharmacy.
  3. Define the core elements of community-based MTM.
  4. Identify the student pharmacist’s role in MTM.
  5. Evaluate a patient medication list to identify drug related problems and design a medication action plan to resolve issues identified.

Target Audience
The target market for this program is pharmacists.

Instructions for Participation
To receive a certificate of completion that indicates your eligibility for continuing education credit, you must:

  • Review the content of the activity.
  • Successfully complete the post-test (70% or higher).
  • You will have two (2) opportunities to successfully complete the activity.

Your statement of credit will be issued immediately upon successful completion of the post-test and submission of the evaluation.
